Abstract

|
The implantable intrathecal drug delivery pump has been widely used for patients with severe intractable pain. Though it can be effectively used when there are side effects or tolerance caused by using high-dose opioids, in a patient with an unstable condition, such as sepsis or organ failure, one might be hesitated to use such an invasive procedure. Nevertheless, terminal cancer patients are often unstable and many patients are still suffering from pain without the help of the procedure.
In this case report, we report an intrathecal drug delivery pump insertion in a terminal cancer patient with repeated acute renal failure and sepsis. The procedure was performed under general anesthesia, with careful attention to the various catheters on the patient¡¯s body which could potentially be obstructed or cause surgical field contamination. After the procedure, the patient showed significant pain reduction, reduced opioid intake, and improved quality of life without specific side effects.
